Medical cannabis is legal in Utah. The state's medical cannabis program was established in 2018 and became operational in 2020. Adult-use (recreational) cannabis remains illegal.

Medical Marijuana Card Process

Step 1

Register with the Utah Department of Health and Human Services' Center for Medical Cannabis.

Medical Eligibility & Consultation Options

  • HIV/AIDS
  • Alzheimer's disease
  • Amyotrophic Lateral Sclerosis (ALS)
  • Cancer
  • Cachexia
  • Persistent nausea
  • Crohn's disease or ulcerative colitis
  • Epilepsy or debilitating seizures
  • Multiple sclerosis or persistent muscle spasms
  • Post-Traumatic Stress Disorder (PTSD)
  • Autism
  • Terminal illness with life expectancy less than 6 months
  • Chronic pain (under certain conditions)
  • Rare conditions affecting less than 200,000 persons in the U.S.
  • Utah allows telemedicine for medical cannabis consultations, including for initial certifications and renewals.

Step 3

Obtain a recommendation from a qualified medical provider (QMP) or limited medical provider (LMP).

Pay the application fee and submit required documentation.

Patient Registration & Eligibility Requirements

  • Valid Utah ID
  • Proof of residency
  • QMP or LMP recommendation
  • Application fee ($15 for initial card, $5 for renewal as of 2024)
  • Create an account on the Electronic Verification System (EVS)
  • Complete the online application
  • Upload required documents, including QMP/LMP recommendation
  • Pay the application fee
  • Await approval (typically processed within 15 business days)
  • Obtain a new recommendation from a QMP or LMP
  • Submit a renewal application online
  • Pay the renewal fee ($5 as of 2024)
  • Renew every six months
