Step 1

Obtain a written certification from a licensed physician

  • Glaucoma
  • HIV/AIDS
  • Cachexia or wasting syndrome
  • Severe chronic pain
  • Intractable nausea or vomiting
  • Epilepsy or seizures
  • Multiple sclerosis
  • Crohn's disease
  • Painful peripheral neuropathy
  • Post-traumatic stress disorder (PTSD)
  • Any other medical condition or treatment approved by the state legislature

Step 2

Apply for a registry identification card with the Montana Department of Public Health and Human Services

Step 3

Pay the application fee and await approval
