Application Process

Step 1

Obtain a certification from a licensed physician

Complete the online application through the Illinois Department of Public Health

Step 3

Submit required documentation, including proof of residency and a passport-style photo

Step 4

Pay the application fee

$100 for a one-year card
$200 for a two-year card
$250 for a three-year card
Reduced fees available for veterans and individuals receiving disability or supplemental income
